A questionnaire may organize observations, but it cannot diagnose a condition or replace a qualified evaluation.
Before using any assessment tool
- Verify who operates it and where data is stored.
- Review consent, access, retention, deletion, and incident-response terms.
- Ask whether it collects health data or makes automated recommendations.
- Provide a human review and escalation path.
- Test accessibility and avoid mandatory sensitive free-text fields.
